Transition into Adult Life
In Manchester we strive to provide excellent support to young disabled people as they move into adulthood. We want to significantly improve both the experience of transition and outcomes for young people (initially focusing on those aged 16 -21) and their families. Key to delivering improvements is the establishment of an integrated transition team from both children's and adult services, bringing together social care and health staff with responsibility for transition. This team is part of the Manchester Learning Disability Partnership (MLDP) and the Adult Services Department but also has close links with Children's services. MLDP is a well established service that has operated for 16 years. It has built up a sound culture of co-operation and promotion of the needs of people with a learning disability. It operates across health and social care, and adopts a single line management structure for all staff. For this development we are also supporting young people with other disabling conditions.

The Getting a Life programme ran from April 2008 to the end of March 2011, as part of the Valuing People Now employment work. Manchester was a pilot site for the 'Getting A Life' programme. The programme tracked 30 young people in Manchester with the aim of identifying what needs to happen to ensure that young people with learning disabilities leave education and go on into paid employment and achieve full lives. - The Transition support group for families. A group of parents/carers who meet regularly to talk about all aspects of transition to adulthood for young people with learning difficulties. (see leaflet).
