What is a Community Learning Disability Nurse

  A Community Learning Disability Nurse is a registered nurse who has specialist training in working with people with severe and enduring learning disabilities and physical disabilities of varying degrees.

•  TheCommunity Learning Disability Nurse can progress their training to become a specialist practitioner or access higher education to extend the scope of their practise. This would be in area's related to people with Learning disabilities such as: epilepsy, diabetes, continence, behaviour management/conflict resolution, sexual health, and mental health.

•  The Community Learning Disability Nurses are based within one of the 3 multi- disciplinary teams, which are located across the city of Manchester.

•  Community Learning Disability Nurses provide health care management to people with multiple or complex health care needs. We try to focus on preventative care and maximising people's health through the use of health assessment tools and Health Action Plans, which are available to all adults with a learning disability.

•  Community Learning Disability Nurses can provide more specialised assessment and care planning in relation to people with specific conditions or syndromes.

•  Community Learning Disability Nurses have a wide range of training and information available and our main focus is to share this information with learning disabled people, their families and carers, and other professionals within the Primary Care Trust.

•  Community Learning Disability Nurses can also provide support to learning disabled people who experience difficulty managing their behaviours. We are able to undertake behavioural assessment, behaviour management guidelines and complete risk assessments on the individual's behalf.

•  Community Learning Disability Nurses work closely with other professional within each of the 3 multi-disciplinary teams. We also work with voluntary and independent services, generic Primary Care Trust colleagues, such as GP's practise Nurses, Dentists and other generic or specialist health care professionals.

 

Community Learning Disability Nurse teams are able to provide information on the following areas:

•  Health promotion/ Improving health

•  Health care management

•  Assessment of general or specific health needs.

•  Assessment in associated areas: Mental health, behaviour etc.

•  Training packages in learning disability related topics.

•  We work with adults with a severe and enduring learning disability and with young adults, who are in transition from child to adult services.

We believe in respecting learning disabled people's right to INDEPENDENCE, INCLUSION, CHOICE, and INVOLVEMENT IN CARE. This is an on-going process supported by City Council and Primary Care Trust initiatives, which are informed by the Department of Health.
