Community Learning Disability Teams

This Community Learning Disability Team provides health and social care assessment and support to people who are learning disabled and their carers.

This includes:

Social Workers and Care Managers,
Community nurses,
Physiotherapists,
Occupational Therapists,
Speech and Language Therapists,
Psychologists

Each team has a psychiatrist linked to it.
