Community Learning Disability Teams

This Community Learning Disability Team provides health and social care assessment support to people who are learning disabled.

The team includes:

Care managers (social workers),
Community nurses,
Physiotherapists,
Occupational Therapists,
Speech and Language Therapists,
Psychologists

Each team has a psychiatrist linked to it.
