What is the Manchester Learning Disability Partnership?
It is:
People who work for Manchester Primary Care Trust and Manchester City Council Adult Social Care Department
These people work together to support adults with learning disabilities living in Manchester.
We have:-
Houses and flats for people to live in with staff - these are called ‘Supported Accommodation Networks'.
Day centres for people to go to in the daytime – these are called ‘Day Services'.
Houses for people to go to for a break or a short holiday - this is called the ‘Short Breaks Service'.
Carers who visit people in their own homes or who live with their families' - this is called the ‘Domiciliary Support Service'.
Health staff and Care Managers who help people live in their community.
These people work in Community Learning Disability Teams. There are three teams in Manchester.
Carers who share their homes with adults with learning disabilities - this is called the ‘Manchester Adult Placement Service'.
People who help us work better – this is called the Quality, Research and Service Development Team.

What are Community Learning Disability Teams?
These are teams of health workers and care managers.
There are three teams in Manchester.
One for the north of the city - called Beechmount Resource Centre, in Harpurhey
One for the middle of the city – called Chapman Place, in Gorton.
One for the south of the city – called Oakwood Resource Centre, in Northenden
The teams will work with you. It is their job to find out what help you want and need.
It is their job to help you get that help.
